Our four different levels of service were designed to help keep home care services more affordable.

These levels include:

Basic Care

For individuals whose care needs are minimal, or are experiencing the initial stages of memory loss, dementia or Alzheimer’s.  CarePlus’s Basic Care services are provided by a specially screened Home Health Aide.   To learn more on how caregivers are screened and seleed, go to Our Caregiving Staff. 
 Basic Care services can include:

  • Assistance with bathing or dressing
  • Assistance with walking
  • Light housekeeping
  • Meal preparation
  • Escorting to appointments
  • Incontinence care (urinary)
  • Meal preparation
  • Medication reminders

Basic Care services are billed at an hourly rate with a 4 hour minimum.

Specialized Care

Individuals who need a higher level of care, are in advanced stages of dementia or Alzheimer’s, suffer from a spinal cord injury or traumatic brain injury, needed assistance with transferring, or are more medically complex would  benefit from CarePlus’s Specialized Care Services.  Specialized Care services are also provided by  Home Health Aides with a wide range of experience in caring for more medically complex clients.  Besides the services provided in Basic Care, our Specialized Care package provides for:

  • Transferring (from bed to chair) with possible use of lift devices such as a hoyer lift.
  • Bowel incontinence
  • Hospice care
  • Post–operative care

Specialized Care services are billed at an hourly rate with a 4 hour minimum.

Skilled Nursing Care

Although most of the CarePlus clients need the services that are provided in our Basic and Specialized Care services…there are still a significant amount who, in addition to these, would benefit from the services of the CarePlus Nurse.
Nurses have gone through significant training and schooling and are taught to observe and identify adverse reactions to medications and changes in the client’s condition, which go beyond the cope of training received by home health aides.  Some of the services provided by a CarePluss nurse are:
Although many elderly clients will have services provided through Medicare,  with cutbacks on Medicare spending there still are many areas where Medicare will not pay for nursing services, and CarePlus can help.

  • Skilled Nursing Visit: 
    • Family members or the client can request a nurse to visit to assess the client’s health status.  These visits are for non-emergency situations only.
  • Medication Set-Ups:
    • Although a home health aide can remind the client when to take their medications, only a nurse can actually set up a medication schedule based on what the client’s physician has prescribed.  Some clients have very complex schedules of medicines, frequently changing dosage or prescriptions. Having a CarePlus nurse to come in and routinely set up the medications for a week or two week period, can eliminate the worry of medication errors. 
  • Dressing changes or administering of other treatments prescribed by the client’s physician:  
    • Many of these more complex need to be done under the supervision of a nurse and a physician’s plan of treatment.  The CarePlus nurse can be an important factor in communicating back to the physician the client’s progress.

Skilled Nursing care is billed at a “Visit Rate” or an hourly rate

Care Management Services

Finding the right services for an aging loved one can be difficult.  Often, you do not know exactly what kind of help you may need, how to find and arrange for services, and how much you will have to pay for services.  These service needs can go well beyond the need for home care services, particularly when family members work during the day or live at a distance away from their aging loved one. Care Plus’s

Care Management services can be a valuable option for older persons and their families.

The Care Plus Care Manager can help put together “the whole picture” of care and service needs for the client and their family, and provide valuable monitoring of   services in place.  Besides their ability to assess for service needs, the Care Manager can provide a wide range of resources, including different types of government funded programs for seniors.

Service provided by the CarePlus Care Manager are:

Comprehensive Assessment:  This assessment differs from the a initial opening assessment of the home care nurse or a skilled nursing assessment in that the Care Manage evaluates not only for health status and mobility/functional status, but also provides an evaluation of mental status, socialization, safety issues, home maintenance issues, financial issues (including ability to maintain daily financial records such as a checkbook), transportation issues, nutritional needs, etc.  The Comprehensive assessment always includes recommendations on resources and plan of care, with considerable thought going to obtaining appropriate services that fit within the client’s budget.

Comprehensive assessments are billed on a fee-for-service-basis.

Facility AssessmentAlthough remaining in their own home is usually the first choice for many clients, there are times when clients may choose, or feel they are best served by residing in an Assisted Living Facility or Skilled Nursing Facility.  Finding the right facility to meet the clients needs can be time consuming and difficult.  The CarePlus Care Manager can provide Facility Evaluations to help clients and their families’ choose an appropriate residence. 

Facility Assessments are billed on a fee-for-service basis.

Care Monitoring:   Many families, as well as other professionals such as trust officers or legal guardians, will rely on the CarePlus Care Manager to provide guidance with ongoing care.  This can include developing and overseeing home modification plans (installing ramps, grab bars, etc.) accompanying client on appointments to physician and other professionals.  Care Monitoring can also include assistance with transitioning to a skilled care or assisted living facility, and ongoing monitoring of the client’s care while they reside in the facility.

Care Monitoring services are billed on an hourly basis.
